OPT Accreditation & CMS
Meeting QUAD A’s accreditation requirements aligns outpatient physical therapy clinics with eligibility criteria for participation in the Medicare and Medicaid programs. Completion of Form CMS-855B and CMS enrollment are required prior to application.
