Our Surveyors
QUAD A surveyors have a wide background in various healthcare settings that play a crucial role in the organization’s accreditation process. Their responsibilities include onsite evaluation of healthcare facilities to ensure they meet established standards of quality and safety. This review process gives QUAD A national and international recognition as the global accreditation authority. Through regular training, QUAD A maintains surveyor certification and obtains up-to-date information critical to performing high-quality surveys.
QUAD A surveyors conduct thorough and consistent evaluations of facilities by employing a comprehensive approach that includes direct observations, conducting interviews with both staff and patients and assessing compliance with each standard. This methodology allows them to accurately determine whether each facility meets the necessary requirements.
Quality improvement methods and periodic assessments are crucial for maintaining the integrity of QUAD A‘s accreditation programs. To ensure surveyor effectiveness and reliability, QUAD A consistently evaluates its surveyors through an ongoing quality assurance process. This process includes feedback from facilities, objective performance metrics, and random validation surveys. Surveyors must remain in good standing with QUAD A to continue conducting surveys.

QUAD A Surveyor Responsibilities
- Participate in a survey team whose size and composition is appropriate for the facility
- Conduct a comprehensive and unbiased facility survey following the procedures outlined in the surveyor handbook
- Complete facility surveys in accordance with the QUAD A survey agenda and checklist
- Follow appropriate time guidelines
- Complete all forms and documentation required as provided in the Accreditation Resource Guide
- Return all forms and documentation to the QUAD A central office in the designated timeframe
- Complete a Statement of Deficiency for each instance of non-compliance observed
- Review any facility plans of correction and the supporting documentation to verify all deficiencies are corrected
QUAD A Surveyor Qualifications
- Be a qualified physician, registered nurse, or rehabilitation therapy provider with a current license.
- Submit a current CV that demonstrates the relevant experience and qualifications required to qualify as a surveyor.
- Physicians must be board-certified or eligible by a certifying board accepted by QUAD A.
- Demonstrate two years of experience in a targeted healthcare environment.
- Be proficient in Microsoft Office 365 tools.
- Perform surveys on the QUAD A-issued tablets and the surveyor’s personnel laptop.
- Sign and adhere to the QUAD A Surveyor Agreement, including Surveyor Attestation, Conflict of Interest, Code of Conduct, Surveyor Guidelines, and agreement to participate in an annual surveyor appraisal and review process conducted by the QUAD A Quality Assurance Program.

- Quarterly Surveyor Calls
- General Information Related to Important Policies
Life Safety Code Surveyors Education
- Completing the Basic Life Safety Code: The Survey Process and Principles of Documentation for Life Safety Code exams are a required component for QUAD A Medicare Life Safety Code surveyors upon completion of a Life Safety Code training session.
- Upon approval to become a QUAD A Medicare Life Safety Code Surveyor, training links and the post-test will be provided.

QUAD A also has committee and sub-committee positions, some of which are open to non-board members. These include:
Investigations Committee
- Electronic review of investigations
- Committee conference calls (varies by committee, on average 2-4 one-hour calls annually)
- Participate in specific projects tasked to the committee by the Committee Chairperson or Liaison
- Serve as subject matter expert for your specialty
- Assist in developing reports for the Board of Directors
Standards Committee
- Electronic review of specialty-specific standards as sent periodically by Committee Liaison during the Standards Overhaul
- Committee conference calls (varies by committee, on average 2-4 one-hour calls annually)
- Participate in specific projects tasked to the committee by the Committee Chairperson or Liaison
- Serve as subject matter expert for your specialty(ies)
- Assist in developing reports for the Board of Directors
Social Media Sub-Committee
- Providing the Sub-Committee with periodic reports related to marketing outreach and campaign performance
Education Committee
- Ensure coverage of appropriate educational topics and content
- Discuss topics of interest, and
- Identify potential gaps in the content calendar or topic areas.
