QUAD A Surveyors

QUAD A Surveyors are certified, expert medical specialists, including rehabilitation therapy providers and outpatient physical therapy specialists, dedicated to the peer-based accreditation process.

QUAD A has gained international recognition as the global accreditation leader thanks in large part to the expertise and diligence of our surveyors. Through regular training and ongoing education, QUAD A surveyors conduct thorough accreditation surveys that increase patient satisfaction and put safety at the forefront.

Surveyor Requirements

QUAD A surveyors evaluate each facility seeking accreditation through observation, staff and patient interviews, and assessment of each standard to verify compliance. QUAD A surveyors must:

Participate in a survey team appropriate to the size and composition of the facility

Conduct a comprehensive and unbiased facility survey following the procedures outlined in the surveyor handbook

Complete facility surveys in accordance with the QUAD A survey schedule and checklist

Participate in a survey team appropriate to the size and composition of the facility

Follow appropriate time guidelines

Complete all forms and documentation required as provided in the Accreditation Resource Guide

Return all forms and documentation to the QUAD A central office in the designated timeframe

Complete a Statement of Deficiency form for each deficiency discovered during the survey

Review any facility plans of correction and the supporting documentation to verify all deficiencies are corrected

QUAD A Surveyor Qualifications

Surveyors are an integral part of the QUAD A accreditation process. QUAD A consistently evaluates its surveyors through an ongoing quality assurance process that incorporates facility feedback, objective performance metrics, and random validation surveys. Surveyors must remain in good standing with QUAD A to continue performing surveys. All QUAD A Surveyors qualifications and duties include:

Be a qualified physician, registered nurse, or rehabilitation therapy provider with a current license.

Submit a current CV demonstrating the relevant experience and qualifications required to qualify as a surveyor.

Physicians must be board certified or eligible by a certifying board accepted by QUAD A.

Nurses are required to have a bachelor’s degree

Demonstrate two years of experience in an OBS setting.

Be proficient in Microsoft Office 365 tools.

Perform surveys on the QUAD A App and the surveyor’s personnel laptop.

The Surveyor position requires strong interpersonal, communication, and critical thinking skills, along with demonstrated expertise in conducting interviews.

Sign and adhere to the QUAD A Surveyor Agreement, including Surveyor Attestation, Conflict of Interest, Code of Conduct, Surveyor Guidelines, and agreement to participate in an annual surveyor appraisal and review process conducted by the QUAD A Quality Assurance Program.

Attend QUAD A surveyor in-service training and pass the Surveyor Training Examination at the conclusion of each training course. A certificate of completion will be kept on file in the QUAD A office.
